• To be completed prior to June 1, 2013.
• Payment for the registration of delegates MUST accompany this form.
• Teams for the upcoming season MUST be registered and paid for in order for delegate registration to be accepted.

OMHA By-Law One 7.3(b) Voter Eligibility 
Each Association in good standing shall be entitled to send to each meeting of members one Delegate for each team eligible for an OMHA Championship. In addition, each Association which has House/Local League Teams, and each Affiliated Independent House/Local League organization, shall be entitled to one Delegate for every twenty (20) teams or part thereof. An Associations' Delegate(s) must be of legal age (18) and must be a member of that Association. Each Delegate shall be entitled to one (1) vote.

The cost is $45.20 per delegate.
